Role
As the Clinical Care Coordinator, you will take the lead in overseeing and coordinating clinical care, guiding a dedicated nursing team to deliver the highest standards of care—all with the strong backing of a supportive Care Manager and Facility Manager.
Responsibilities
- Provide clinical leadership, support, coaching and mentoring to staff
- Collaborate with other healthcare professionals to ensure the effective development and delivery of appropriate care plans
- Ensure compliance with the facility's documentation requirements
- Maintain clinical governance, supervise infection control protocols, and oversee medication management
- Foster strong relationships with residents, families, and stakeholders to promote optimal health outcomes
Key Candidate Requirements
- Tertiary qualification in Nursing with AHPRA Registration
- Full Australian working rights
- Strong knowledge of regulatory requirements, care governance, and clinical standards in aged care
- Proven ability to lead teams and manage complex care scenarios in an aged care environment
- Demonstrated understanding of AN-ACC assessment processes
- Previous experience working within a Residential Aged Care setting in a similar role
- Excellent communication and problem-solving abilities
- You’re a proactive, solutions-driven thinker with a passion for clinical excellence
