About the Role As the Clinical Care Coordinator, you will play a pivotal role in ensuring residents receive safe, evidence-based, person-centred care. You’ll work closely with the Care Manager and multidisciplinary team to oversee clinical governance, support nursing staff, and lead continuous improvement across the home.
Key Responsibilities
- Oversee day-to-day clinical operations across the home
- Provide hands-on clinical leadership, mentoring and support to RNs, ENs, and carers
- Coordinate resident assessments, care planning, reviews and documentation
- Ensure compliance with the Aged Care Quality Standards and clinical best practice
- Lead clinical audits, incident reviews and quality improvement initiatives
- Liaise with residents, families and external health partners
- Support rostering, skill-mix planning and onboarding of new clinical staff
- Act as second-in-charge for the Care Manager and provide cover as required
- Registered Nurse with current AHPRA registration
- Strong clinical assessment, decision-making and organisational skills
- Previous experience in aged care or subacute care (preferred)
- Confident leading teams and supporting staff performance
- Excellent communication skills and a commitment to person-centred care
- Knowledge of Aged Care Quality Standards, SIRS and contemporary clinical practice
