About the Role
As a Community Registered Nurse, you will provide clinical care, assessment, and case management support to older Australians living independently in the community
Key Responsibilities
- Conduct comprehensive clinical assessments
- Develop and review care plans
- Medication management and wound care
- Chronic disease management
- Clinical documentation and reporting
- Liaise with GPs, allied health, and external providers
- Provide education and support to clients and carers
- Ensure compliance with Aged Care Quality Standards
- Current AHPRA registration as a Registered Nurse
- Minimum 2 years’ post-registration experience (community/aged care highly regarded)
- Strong assessment and care planning skills
- Excellent communication and time management
- Ability to work autonomously
- Current driver’s licence and reliable vehicle
- National Police Clearance & relevant clearances
