In this role, you will provide clinical oversight, case management and coordination of care to support clients to live safely and independently in their own homes.

Key Responsibilities
- Manage a portfolio of home care clients, delivering end-to-end clinical case management
- Complete comprehensive assessments, care plans and regular clinical reviews
- Provide clinical oversight and guidance to care staff and support workers
- Ensure care delivery meets Aged Care Quality Standards and Home Care Package requirements
- Monitor clinical risks, incidents and escalation of care as required
- Liaise with GPs, allied health, families and external stakeholders
- Maintain accurate documentation and reporting in line with compliance requirements
- Current AHPRA registration as a Registered Nurse
- Experience in home care, community nursing or aged care
- Strong clinical assessment and care planning skills
- Sound knowledge of Home Care Packages and aged care compliance
- Excellent communication, organisation and time-management skills
- Ability to work autonomously while contributing to a collaborative team
